Scheduling Your New Patient Appointment
We often layer services for our patients to receive as a part of their treatment plan. Most services are also offered as a stand-only treatment option. These services can be broken down into 6 major categories: Chiropractic Care, Physical Therapy, Massage Therapy, Acupuncture, Functional Medicine, and Personal Training.
Chiropractic Services: We get to the root of the problem causing you pain with a treatment plan personalized to your needs. We are a family practice with a no-nonsense approach to your care, helping us better serve our patients that ranges from infants to seniors with all sorts of conditions and injuries. We are certified in the patented Webster technique as well as Selective Functional Movement Assessments and Active Release Technique.
Physical Therapy: Our physical therapy program utilizes a comprehensive approach that focuses on creating a stretching and exercise routine you can perform at home that will help restore your strength, flexibility, and stability.
Massage: Specializing in seven types of massage therapy including prenatal and sports massage, Ravenswood Chiropractic & Wellness Center’s licensed massage therapists can help you achieve physical wellness.
Acupuncture: Promoting physical and emotional health, acupuncture has been used for centuries to relieve a wide variety of health and wellness issues. Our holistic acupuncture therapy approach is based on proven, traditional methods.
Functional Medicine: We are what we eat, and an approach to wellness that includes a focus on healthy eating habits as well as physical fitness can have numerous benefits. We will help you get there by supporting a healthy diet based on whole foods, regular checkups, and natural interventions. We also offer comprehensive testing including food allergy/sensitivity, digestive, and hormones.
Personal Training: We’ll get you in shape with our personal training staff who are at the very top of their field. We offer private or semi-private personal training sessions that will teach you simple, fun exercises that’ll lift your spirits and get you moving.
Our patients can schedule appointments in person or over the phone just like they could in any medical office.

If you have been seen in our office before you should have received a personal link to your online scheduling portal, if you need assistance or require a new link please let us know by calling us at 773.878.7330 or submitting your request here.
You may cancel or reschedule your appointment without charge if it is more than 24 hours’ notice. If you cancel your appointment within 24 hours of your appointment, you will be charged a $45 fee per scheduled service (not to exceed $90).

You will be asked to fill out New Patient Forms which will help us learn about you and your condition. Please take your time to fill them out completely. The more information you are able to provide, the better we will be able to help you. You will be sent the forms to fill out digitally once you schedule your first appointment, or you can wait to fill them out once you’re here. All forms will remain strictly confidential (see our privacy policy for more information).
The doctor will ask you questions to assess your condition and how it is affecting you, and then will proceed with an examination. Nothing will be done in our office without your consent.
You will then receive a thorough physical exam which will include standard orthopedic, neurological, postural, and other examinations as needed. After the initial Examination, the doctor may order additional testing to be performed. The doctor will do this to determine the cause of your condition. No additional testing will be performed in our office without a full explanation of the procedure, your options, and your consent.
Additional Testing which may be ordered may include: X-ray, CT, MRI, Ultrasound, EKG, Stress Test, or Laboratory Testing. If you have recently had testing done, please bring in copies of the films, reports, and laboratory results to your first appointment along with a list of your current medications (include herbal and over the counter).
After the doctor has gathered and analyzed your information, examination, imaging, and testing results, the doctor will review his findings with you and explain recommendations for treatment.
A spouse or family member is always welcome to attend, but in light of COVID-19, please let us know beforehand if someone else is coming to your appointment with you.
Masks are required: All team members and patients are required to wear masks while inside our office.
Hand Washing and Hand Sanitizer: Use of the sinks located in the bathrooms or hallway outside the Physical Therapy Room is encouraged. Hand Sanitizer is available for your use in the reception area.
Screening: All team members and patients will have their temperature taken upon arrival and asked questions regarding their recent health history before arriving at our office.
Cleaning: Our team cleans treatment rooms with medical-grade disinfectant after each patient visit. Common touch points and surfaces throughout our office are cleaned hourly with the same medical-grade disinfectant. Extensive cleaning of the office is done at the end of each day.
Physical Distancing of at least 6 feet from others is required whenever possible: To encourage this we have significantly reduced the number of appointments available and the number of people permitted inside our office. This means our team is limited to essential team members only and patients are not permitted to bring guests with them to their appointment at this time unless they are a parent | legal guardian or a caretaker for a patient that requires support.
We ask that you arrive to your appointment on time which will help us to not have anyone waiting.
Filling out any required paperwork ahead of time will help minimize the amount of time you spend in the office. Our patient services director will coordinate any of these needs with you before your appointment.

After your Chiropractic Exam, you and your doctor will form a treatment plan personalized to your needs. No two patent’s treatment plans are exactly alike, so this is a collaborative process between you and your doctor in which they state what you need in order to get well, and you state what you can commit.
Once your treatment plan is established, you’ll receive your treatments, and the doctor will continue to evaluate your condition and track your progress until you get better.
Paying for Your Care
We accept most insurance plans, as long as it’s a PPO. Listed below are some of the providers that we accept.

We accept insurance plans from many providers that are not listed above. If you have a PPO insurance plan that is not listed above, if you have questions about your policy, or would like your insurance verified before your first visit, you can submit your insurance information here and we will verify your available coverage in our office and our patient services coordinator will review it with you. If you have any questions you can call us at 773.878.7330 for assistance.
Unfortunately, we do not accept any HMO, Medicaid, or AllKids insurance plans.
If you do not have insurance or our services are not covered by your plan we offer ChiroHealth USA.
ChiroHealth USA is a discount membership program that allows us to give you a discount off our regular fees. It provides a minimum discount of 33%+ and provides a maximum fee for Chiropractic treatment with or without an examination. There is an annual fee of $49 paid directly to ChiroHealth USA which covers the purchaser and their spouse and or partners and dependents for one full year. The cost of the membership fee is usually saved in one visit. We receive no compensation from ChiroHealth USA.

Patients may cancel or reschedule their appointments without charge provided they give us at least 24 hours notice. If you cancel your appointment within 24 hours of your appointment, then we will charge you a small fee.
